अन्तकाले च मामेव स्मरन्मुक्त्वा कलेवरम् । यः प्रयाति स मद्भावं याति नास्त्यत्र संशयः
antakāle ca māmeva smaranmuktvā kalevaram | yaḥ prayāti sa madbhāvaṃ yāti nāstyatra saṃśayaḥ
Translation
And whoso, at the time of death, thinking of Me alone, leaves the body and goes forth, he reaches My being; there is no doubt in this.

Śaṅkarācārya's commentary
"Me": Viṣṇu, the Supreme Lord. "My being": My real being as Viṣṇu. "In this": as to whether he reaches or not.
